What type of services do you offer?

Our therapy staff provides care for individuals suffering from pain or disability injuries sustained at home, at the workplace, sports related, auto accident injuries, preoperative and postoperative patients, pediatric or developmental conditions, and adult onset neurological conditions.

How often will I come to therapy?

The number of visits vary depending on your injury. The physical therapist may wish to treat you 1-3 times per week based on the initial evaluation and referring physician’s suggestion.

How do I know if my insurance covers physical therapy?

Click Here for the list of insurances that we accept.  We will work with all insurance companies that we are non-participating with through your out of network benefits.  To confirm your physical therapy benefits please refer to your policy manual or call the phone number on the back of your insurance card.

Is there a co-pay?

Every plan is unique.  Please refer to your policy manual or call the phone number on the back of your insurance card for your outpatient physical therapy benefits, to confirm.  All copayments are due at time of service.

Can I pay with my credit card?

At Family Physical Therapy we conveniently accept Visa, Master Card, Discover, and American Express in addition to check or cash.

I can’t afford this. What can I do?

We understand that anyone can have financial hardships, which is why we have a few options available for those who may need them.  Ask us about a payment plan that we can set up so you can still obtain all of the benefits of therapy that you may need.   Or, ask your therapist about increasing the home exercise program and decreasing the number of visits.  Although we believe the full amount of visits will increase your recovery rate, we will do our best to work with your individual needs.

How do I request physical therapy?

If you have an injury or illness that affects your normal pattern of activity, contact your Primary Care Physician (PCP) for an evaluation.  If you do not go to your PCP first, we will be in contact with your physician to obtain the referral to ensure insurance coverage.

What paperwork should I bring?

New patien paperwork (Click Here to print out the patient forms), a photo ID, your current insurance card, referral (if applicable), auto insurance information (if applicable) and workers compensation information (if applicable). Please be sure to bring all documentation to your initial evaluation.

What is done on the first visit?

You and your physical therapist will discuss your current condition and medical history.  Your therapist will then check your range of motion, mobility, function, balance, strength, gait, ect.  You may be given some home exercises to help reduce pain and improve mobility.  Your physical therapist will then educate you further about your condition regarding posture and activity modification to ease pain.

What should I wear for my visits?

You should be wear comfortable clothing.  Be prepared to exercise and/or stretch as needed, keep in mind you may need to expose the body area for which you are seeking treatment.

How long are the visits?

Your physical therapist will spend about an hour with you for your initial evaluation, and follow up appointments usually take between 30 & 60 minutes depending on modalities performed.

Until I see the physical therapist, should I use heat or ice on my injury?

In most instances, ice is better after an injury, when there is inflammation, and/or muscle spasms.  But keep in mind every case is different, your physical therapist will know what’s best for you after your initial evaluation.
